Eligibility

Active, regular, full-time Gongsters working a minimum of 30 hours per week are eligible for the benefits outlined in this guide on date of hire. You may also enroll eligible dependents in certain benefits.

Part-time, temporary and seasonal Gongsters and their dependents are not eligible to participate in the benefits described in this guide.

Eligible Dependents

  • Your legally married spouse;
  • Your same-sex or opposite-sex domestic partner (DP);
  • Your dependent children up to age 26 (regardless of marital status), including a natural child, stepchild, a legally adopted child, a child placed for adoption or a child for whom you or your spouse are the legal guardian;
  • Your unmarried children aged 26 or older who are mentally or physically disabled and who rely on you for support and care; and/or
  • Children of a same-sex or opposite-sex domestic partner (DP) relationship, up to age 26 (regardless of marital status).

Documentation Verification for Dependents

In order to cover your family members, Gong needs to verify dependent eligibility. If you are adding a dependent, you must provide the following documentation to complete enrollment:

  • Spouse: Marriage certificate
  • Child: Birth certificate or other proof of birth
  • Domestic Partner: Declaration of domestic partnership

Domestic Partner Coverage

IRS regulations mandate that the value of the health care benefits are considered taxable income to the Gongster. Contributions for the domestic partner and domestic partner’s children need to be made on an after-tax basis.

Domestic Partner (DP) Eligibility Criteria

If you are enrolling a Domestic Partner, you affirm that you have met all eligibility requirements listed below for the previous 12 months:

  • You maintain the same principal place of residence and intend to do so in the future.
  • You agree to be responsible for each other’s basic living expenses if either of you is unable to provide such expenses for himself or herself.
  • You are both 18 years or older.
  • Neither of you are married.
  • You are not related by blood to such a degree that you would be prevented from marrying in the state in which you reside.
  • Neither of you have maintained coverage for another Domestic Partner under any health plan within the last six months (this excludes a Domestic Partner that has died within the last six months).
  • You agree to notify Gong immediately upon your inability to satisfy any of the criteria of Domestic Partnership.
